I think there is a mistake in the crash location. The plane crashed a mountain called \"Rocher des Deux soeurs\". The place indicated here, the village of Château-Bernard which includes a mountain \"Rocher des Deux Soeurs\", 5°35\'E and 45°00\' N Alt 2193m but it is wrong. I think the crashed location takes place 20km west, on the western part of  the Vercors Plate. There is an another mountain called \"Rocher des Deux Soeurs\" 5°11\'E and 44°56\'N Alt 1330m.
I can say this because near this place there is a pass called \"Col de Tourniol\" (1145m) where there is a stele remining this crash with a plane propeller.  The Location you should indicate is 10km South-East of Romans-Sur-Isère, the most well-known city near the crash location.

You must be a TAC ex-fighter pilot. The KC-135 did NOT TURN INTO THE F-105.... He, the F105 pilot was number ###4 IN A FLIGHT OF 4 F-105\'s and he was simply following \"in tight formation\" his Flight Commander. The formation of four F-105\'s came busting into and AT THE SAME Altitude as the KC-135\'s ASSIGNED ALTITUDE. They, TAC, At that time did not feel they had to follow SAC Safety procedures for \"JOIN-UP\". The Flight Commander  in charge of the TAC 4, killed one of his own men and DOWNED a KC-135 with 4 crewmen on board. I wonder if the Flight Commander bothered to put a \"KILL\" star on his personal bird....Floyd

He died due to complications from the severe burns he received in the crash.

As far as I know, the plane was arriving in La Paz in snowy weather loaded with meat from Santa Ana.  The plane became heavier because of the snow and crashed 2 minutes before landing at El Alto.  Walter Ballivian was the pilot.  The airport runway was cleared waiting for him to land safely.  He was requested to divert to Cochabamba because of bad weather but refused to do it.

I would like to add to the accidents occured to Aerolineas Argentinas one happened in july 31, 1986 to a Fokker F-28 registration LV-LOA that skidded out of the runway while landing during a snowstorm. The Argentine Air Force intentionally destroyed almost every evidence of the accident. However I still have the pictures and some of them can be found in Airliners.Net.

Well, a topic that has been flying around already after Iberia\'s EC-JOH\'s overshoot on RWY 35 in UIO back in november, has now come to an \"end\". Yesterday, a friend of mine, in charge in the repair analysis, and the possible repair itself, wrote me an email. He told me he was back in Ecuador but for bad news. He told me that the final decision has been taking (I\'ll avoid details) and Iberia EC-JOH will be scrapped starting today. I thought you guys might like to know this, because it is a sad, and unbelievable decision, we weren\'t expecting of course. Just to think a 2 year old plane will be cut, is amazing. I have spoken with him and there is a chance to help and take pics of this event, so I hope to be able to document this, and maybe take some pics. Anyway, thats all for now, im still wondering how they will deal with the pieces, maybe they\'ll take them in a freighter for spares in TLS, or maybe destroy it all, i don\'t know, but still. Ill keep you posted. Thats all for now. Ohh, yes, the process will be finished in 20 days just about.

Airplane damage - written off (broken up for spares Alek Feb07).   This aircraft had earlier been damaged when landing at Wunrok having struck trees as it landed in Oct06.  It was repaired on site and this final flight was only its second flight for Astral, the first having been frin from Nairobi International.  There were reportedly just two crew on board when it suffered its final demise.

The wreck lies just beyond the end of RWA 20 in the  grass of the safety zone. Perhaps the location can be indicated by an arrow on the aerial photograph, indicating the end of RWY 20 ?.

Accident site is quite far off pointed in google maps.
It should be placed north of the A8 highway and the town \"Durango\", between \"Maguna\" and \"Gerea\", where you can see shadows of a few wind-energy towers.  At the left of these, is the TV antenna where the port front end and left wing hit while banking left, at 3356 ft. Over and to the right is a dark pine-tree area where the rest of the aircraft plunged upside down, that\'s the northern slope of Mount Oiz.

On December 26th, 1982 an EA-6B of VAQ-129 crashed at KNRA (US Navy, Whidbey Island Naval Air Station, Coupeville Outlying Field while performing Field Carrier Practice Landings. I was a witness to the accident (which killed 3 crew) but never heard what the cause of the accident was.

Runway length off the Jepp plates is 6,109 feet total length, but there is a displaced threshold for landing that shortens the available landing distance to 5,410 feet.  Runway 02 also has a downhill slope of about .475 percent.

(3,300 ft. airport elevation + 5,410 landing distance + downhill runway slope + 9 or 10 knots of tailwind, plus a full aircraft = a nasty deal)

The aircraft had taken off from Ballykelly in Northern Ireland on an anti-submarine exercise early on the morning of the 19th April 1968.  The area that day was shrouded in thick sea mist and at about 10:30 the aircraft was heard in the area south of the Mull of the Kintyre shortly after as it climbed up through the mist the aircraft struck rising ground near Garvalt cottage killing all 11 crew members instantly.

WD483 crashed on landing at Ataq on the 9th April, Peter King has asked me to post this:
Seeing 09/04/1956 made me remember that I had loaded that aircraft at Khormaksar during the night before it crashed at Ataq. 
 
As I recall, WD483 was from 70 Sqn (at that time probably at Nicosia) and was originally on a scheduled passage Nicosia - Khormaksar - Eastleigh (Kenya). It had arrived at Khormaksar in afternoon of 8 April and would probably gone on next day to Eastleigh, quick turnround and back to Khormaksar that day and next day return to Cyprus.
 
I remember that in the weeks prior to this there had been \'trouble\' up-country and that an army unit had arrived in Aden, presumably to sort it out (remember Aden didn\'t, at the time have a permanent army presence. I know we had numerous army Sergeants sleeping on the verandah outside my room (I had recently been promoted Sgt and ran one of the two shifts at Air Movements).
 
When we took over on evening of 8 April we were told that we had to unload the 70 Sqn Hastings of its load (intended to go to Eastleigh next day) and load a Ferret Scout Car which was to go to Ataq where we assumed the army would use it (ours not to reason why ......!!)
 
First problem was that to load vehicles into  a Hastings you needed a ramp and at Khormaksar we only had a single \'heavy-duty\' version. OK for us but what about at Ataq? Now there were light duty ramps, air-portable so we understood though we had never seen one so we were told that workshops airmen would then dismantle our ramp, load it into aircraft behind the ferret, take it off at Ataq and re-assemble it to use there!
 
I know that by about midnight after much sweating and exhaust fumes the ferret had found its way up the (was it 11 degree?) slope and we had it safety chained down. Now the workshops staff set to work on our one and only ramp and reduced it to a number of bulky, heavy, pieces. Eventually we got this on board and TASF refitted seats behind it for the ferret driver and crew and for several workshop staff who had to put it together at Ataq. I was told that I did not need to worry about the Weight and Balance Sheet (old-fashioned version of trim sheet) since the AQM would do his own, also no Movements staff were need to accompany aircraft as AQM would release chains etc. Source http://splashdown2.tripod.com/id11.html

“My other points relate to safety. 

If you’re an experienced jumper then it’s your call if you get on a load that the DZ is sending up in questionable weather. However I did witness a load including students take off when a storm front was coming in. The drop zone was playing the game of averages and this time they got unstuck as the result was a student who could not descend for some 25minutes due to winds and updraft and who landed some 3-4km’s away from the DZ. 

He was OK in the end but this story could have been very different due to poor decisions by the DZ operators. “

“Cumulonimbus clouds contain severe convection currents, with very high, unpredictable winds, particularly in the vertical plane (updrafts and downdrafts). They are therefore extremely dangerous to aircraft. Smaller, propeller-driven planes cannot cope with the conditions and must fly around them; larger jet aircraft fly over the smaller ones and around larger examples. Larger planes are also equipped with weather radar and wind shear detectors to help guide them through, in the event that they need to pass through such clouds to land. They also can snow because they are also in the higher part of the atmosphere. \"

Towering “cbs are the most dangerous of all as they signify well developed storms, easily visible with the most erratic of conditions. 

These types of clouds do not randomly appear they give warning signals well in advance (45 minutes), besides which they existed for the most of the day as the Madrid airport records indicate so they did not come out of nowhere.

The remains of the cloud can be seen drifting off on the video here. Also note the ground is wet suggesting recent rain with no dry patches. Rain is a feature of a CB cloud. This appears to have been some time, 1 hr + after the accident.

Second 26: Witness describes seeing the airplane intact spinning out of control and seeing the skydivers jump, followed by the wing snapping.  (Under the suspected overload for which the aircraft was not designed.) 


Second 50 : You can see water drops on the lens of the camera ,the wet ground and tracks made with mud on the wheels of the rescue cars.  Look at the umbrellas.

Second 56: The more general conditions as The reporters were making their way to the scene. (Closer to the time of  the accident ) . The place had a downpour one would associate with CB clouds. Rain was not forecast / observed  to come from any other type of cloud.


Injured people 

The people injured , the ones who jumped were students ( tandem ) . The question really must be asked , if these Towering CBs are dangerous to large aircraft at large distances what business does a center have sending up relatively unprotected parachutists.

\" Better to be on the ground wishing you were up there , than up there wishing you were on the ground \" . Yet people  \"trust”   the centers managers  to make correct choices in regards to their safety. See the above review.




Speculative Chain of events :

1. Strong thunderstorm activity since noon. 
2. Pilot on duty with little  in cloud flying experience ( El Mundo )  . 
3. Tandem passengers arrive ( people on board were all staff ( camera etc.  or students ) 
4. Despite the poor weather picture, pressure is felt to jump the students.
5. Well developed and multiple towering CBs on the horizon. 
6. Airplane takes off.
7. Another aircraft inbound to the field views the conditions as too bad and turns away. ( CNBC ) 
8. At this instant it is clear that the plane is moments from drop
.9.  Jumpers report the plane lurching violently in cloud and having to walk along the roof to get out. 
10. Pilot has completely lost control of aircraft, inverted and outside of its certified limits it ultimately destructs due to the great stress on the rebuilt airframe. ( Even if it was new its not likely it would have tolerated the tossing around from a towering CB cloud. 


How could this have been avoided?

A flight should not have gone in these conditions.  Perhaps they thought as in the case of the above review that they would “ make it “. 

There may be some inaccurate items here but I have reasonably satisfied myself of the validity of my theories and would be surprised if the official reports differs by much

The dispatch from Pilatus reference the inspection was made as a precautionary measure ?? These parts are subjected to non destructive testing ( ultrasound ) on a regular basis..

Los Angeles Airways Flight 841 crash in Paramount, California.     Some info from the wikipedia page on Los Angeles Airways Flight 417:

This was the second fatal crash of a Los Angeles Airways Sikorsky S-61L helicopter. The first one occurred only three months earlier on May 22, 1968, when N303Y was cruising at about 2,000 AGL, after departing the Disneyland heliport, and while proceeding to LAX. At about 5:50 PM, Flight 841 suddenly broadcast a distress message, near Paramount, California: \"L.A., we\'re crashing, help us!\" As in the subsequent crash of N300Y, there were no survivors among the 20 passengers and 3 crew. The aircraft was destroyed by the impact and subsequent fire. That crash was also caused by a mechanical failure in the blade rotor system, which then allowed one blade to strike the side of the fuselage. The other four blades were then thrown out of balance and all five rotor blades broke and then the rear fuselage and tail separated from the rest of the airframe.

The \'pilot\' was one LAC Nick Agnani, a Parsee Indian of British citizenship serving in the RAF as a groundcrewman, his colour-blindness having failed him as potential aircrew — something that nagged at him, according to memories of colleagues. He was based at RAF Thorney Island and is said to have taken two flying lessons at nearby Portsmouth Aero Club, whose CFI, Sqn Ldr Harry \'Mitch\' Mitchell advised him not to waste any more of his money as he lacked aptitude. Agani reportedly spent much of his off-duty time alone in his billet, reading aircraft technical and navigation manuals. On the evening in question he got aboard a 2 ANS Varsity, WF426, started it, made unsteady progress to the runway, and took off, passing very low over the sea wall where some of his fellow airman were cockling. Somehow he crossed the Channel, but crashed into a farmhouse at Onnaing, near Valenciennes in northern France, killing himself and three people in the house. I\'ve seen an eyewitness report that prior to stealing the Varsity, Agnani had been caught in the off-limits hangar at Thorney which housed a detachment from the Aerial Torpedo Development Unit at Gosport, said then to have been operating three Canberras. Some say he was homesick and planned to fly back to his family, others that he simply wanted to prove that he could be a pilot. Either way, it ended tragically.

In 1987, a Continental Airlines 727 Departing hit a Red Tailed Tropic Bird, causing loss of the left pod engine; fuel was dumped, and airplane made an emergency landing at Johnston.  Passengers and Crew were stranded.  Military aircraft flew people back to Honolulu; Continental Air Mike sent a repair crew from the Guam base the next day with a replacement engine; this passenger (rear)/cargo (front) 727 went to Honolulu, picked up previous day\'s passengers, and returned to Johnston.  Repair crew informed inconvenienced poassengers that it was a sick bird, and its stomack/gullet was full of rocks that destroyed the turbine, causing the accident.  Passengers continued on to Kawajelein on replacement aircraft.  Repair crew replaced engine, and a crew later flew it back to Guam for additional checks.

This definetely was a departing aircraft \"incident\" at Johnston Atoll:  I know: I was on that plane!

The following is taken from \"Avro Shackleton\" by John Chartres.
11 January 1955: Mk 2s WG531 and WL743 of No.42 Squadron. Eighteen killed, no survivors.
In this, the most catastrophic of all the Shackleton accidents, the two aircraft left St. Eval at 10.14 and 10.20hrs respectively to carry out a 15 hour patrol and search exercise to the south of the Fastnet Rock off Southern Ireland.
The first aircraft off, captained by F/O. George Board was 14 ninutes late due to reasons beyond his control and the second (P/O. Woods) left early, thus reducing the normal half-hour separation time to six minutes. This factor tended to lend credence to the inevitable theory that the two aircraft were lost in a collision, possibly following each other in a \'creeping line ahead\' search pattern or by practising homing in on one another.
The CO of 42 Squadron at the time, S/L. Norman Wilson was never entirely convinced by the collision theory finally recorded as the least improbable, particularly as radio messages were received indicating that the two captains had adjusted their separation and that up to 20.00hrs that night were flying at the prescribed 85 miles distance fron one another.
In the event all contact was lost from 20.58hrs when a ground radio operator attempted to call P/O. Woods with a barometric pressure reading over his exercise area which he had requested earlier. There was no reply from him and neither aircraft made their scheduled 21.00 nor 22.00hrs reports. In spite of an enormous search operation involving other Shackletons, Lancasters, Sunderlands and many smaller aircraft as well as surface ships no trace was found of either aircraft until more than 11 years later when the starboard outer engine of P/O. Woods aircraft was trawled up off the southwest Irish coast, about 75 miles north of the assumed collision point.

Not a correction but a contribution.  Copilot, upon landing, instead of deploying flaps pulled the reversers. Fell like a papaya fruit among a big cloud of dust. They tried to deceive the spanish insurance company. Investigators found the reversers engaged. End of the claim. Thank you.

A note: According to testimony of Mr Oscar Padgett, at that time chief of tarmac services of the Managua International Airport, he warned the pilot of dirty fuel filter. Pilot argued that filter would be replaced at home base in El Salvador because cheaper labor and paying in local currency and not US Dollars. Ended in tragedy. The same applies to TACA flights with bald tires. They will be changed at El Salvador for the same reasons. Thanks you.

A small commercial aircraft departing from Guatemala\'s international airport yesterday (18 September) crashed shortly after takeoff, killing at least seven people.

The two-engine, 20-seat aircraft belonging to Atlantic Airlines took off from Guatemala\'s Aurora airport for a flight to Honduras, but crashed back to the ground. Six people died on the aircraft, while a seventh man died at a hospital. Four other passengers are being treated in hospital for serious injuries.

As the crash occurred on the airport\'s main runway, all other takeoffs and landings had to be halted. It was unclear when the airport would reopen, The Associated Press reported.

This is not a correction, but I feel that the following entry should be researched, rewritten and added to the archive as it does not at present appear here (the text is quoted from Wikipedia).

On October 23, 1999, the actor, Harrison Ford was involved in the crash of a Bell 206-L4 helicopter (N36R). The NTSB accident report states that Ford was piloting the aircraft over the Lake Piru riverbed near Santa Clarita, California on a routine training flight. While making his second attempt at an autorotation with powered recovery Ford allowed the aircraft\'s altitude to drop to 150–200 feet before beginning power up. As a result the aircraft was unable to recover power before hitting the ground. The aircraft landed hard and began skidding forward in the loose gravel before one of its skids struck a partially embedded log and flipped onto its side. Neither Ford nor the instructor pilot suffered any injuries though the helicopter was seriously damaged. When asked about the incident by fellow pilot James Lipton in an interview on the TV show Inside the Actor\'s Studio Ford replied \"I broke it.\" [LAX00LA024. This aircraft is a Valetta T.3 serving with 2 ANS at time of accident, not a Varsity T.3.
WJ470 was part of Contract 6/Acft/5930: WJ461 to WJ487

The Viking and Valetta provided the basis of the Vickers Varsity. The only difference between the Varsity and the Valetta was the position of the third wheel. The Varsity had a nose wheel and the Valetta a tail wheel.

My uncle, mr. Alexandre Rieper Filho is death in the accident. The cause of accident is misterius, but we know the cause is sabotage. Acid cloridic in fuel is the cause of explosion of airplane.

I run a web site, called Iwo Jima Memoirs. I collect stories from service members who served between February 19, 1945 and 1968 when the island reverted back to Japan. 

One individual who was on the island at the time, witnessed the crash of this C-47 after it took off from the island. He knew names of some of the people who were on the plane.

I witnessed the accident, and my father was flying the Shackleton that followed the accident. The Shack was supposed to do a low pass with 1 and 4 feathered They had feathered 1 and 4 engines for effect and on the pull up, the pilot called for start on 1 and 4 - the copilot feathered 2 and 3 with the expected result. - I\'m confirmed this account with my father recently.

Narrative:
The Boeing 707 freighter was making an instrument approach to runway 14 with speed brake lever at 45deg. and inboard spoilers deactivated when it was noted that the spoiler switch cover was open. The captain or co-pilot then closed the spoiler switch cover and thus activating the inboard spoiler, causing the aircraft to pitch down. The Boeing descended fast from a height of 70m, struck approach lights and ditched.

The site pointed here is of the new Denver International Airport. The crash occured just north of runway 35L at the old Stapleton airport, several miles to the west.

I have found the grave of Sgt JF Wallace (Air Gunner) in All Saints Church, Honington, Suffolk. Died 23.2.1941. A search on the Commonwealth War Grave Commission web site says he was from 151 Sqn.

The database says Dutch Continental Airways was founded in 1996 but lists an incident on Aug 30, 1970.

I can\'t find anything on \"Dutch Continental Airways\" and I don\'t remember it from history lessons (I\'m Dutch).
I just know they could not have an incident in 1970 if they only existed after 1996.

This Viscount was deviated from its destination by Castro\'s 26 of July rebels that hijcked the plane at gun point and forced to fly to the Oriente Province. The plane was approaching a little airport of Mayari de Arriba, too small for this plane, when it crashed into the Nipe Bay. This was the first airplane hijacked in the world
