Détails:Three engines had lost power shortly after take-off from runway 10. The aircraft crashed just 2km short of runway 19 in an attempt to return to the airport.
It appeared that the fuel was contaminated.
PROBABLE CAUSE:
pilot in command - inadequate preflight preparation and/or planning
personnel - flight engineer: inadequate preflight
personnel - maintenance,servicing,inspection: improperly serviced aircraft (ground crew)
miscellaneous acts,conditions - fuel contamination-exclusive of water in fuel

personnel - operational supervisory personnel: deficiency,company maintained eqpmt,services,regulation

terrain - high obstructions
partial power loss - partial loss of power - 3 engines
