Narrative:
The aircraft touched down on a wet runway and reverse thrust was applied on all engines, except no. 4 (of which the reverser had been de-activated). When coming out of reverse, the no. 4 engine reached 1.4199 EPR, causing the Boeing to yaw to the left and depart the runway 2400m past the threshold.

PROBABLE CAUSE: Inadvertent application of power on the no.4 engine while coming out of reverse.
