Descripción:
TANS Flight 204 departed Lima at 14:24 on a scheduled 53-minute flight to Pucallpa. After an intermediate stop there the airplane was to continue to Iquitos.
At 14:52 the crew initiated the descent towards Pucallpa with the intention of carrying out a visual approach and landing. While approaching Pucallpa meteorological conditions deteriorated with towering cumulus clouds, strong winds and heavy rainfall. The runway 02 approach was unstabilized, but the crew continued the approach. The GPWS sounded as the airplane penetrated a severe hailstorm. The crew lost situational awareness and failed to abort the descent. The Boeing 737 crash-landed in swampland and broke up. It appeared that a copilot under instruction was in the right hand seat while the regular copilot was in the passenger cabin at the time of the accident.

Probable Cause:

CONCLUSIONES: "La Comisión de Investigación de Accidentes de Aviación del Ministerio de Transportes y Comunicaciones, determina sobre la(s) probable(s) causa(s) del accidente, como sigue(n) a continuación:
1) La decisión de la tripulación técnica de continuar con la aproximación final y el aterrizaje hacia el aeropuerto de Pucallpa en condiciones meteorológicas muy severas (Tormenta).
2) La decisión de la tripulación técnica de realizar el descenso no estabilizado y de no actuar para detener el descenso abrupto hacia el terreno, en régimen de descenso superiores a los 1500 pies por minutos; lo que activa el GPWS (Ground Proximity Warning System).
3) La decisión de la tripulación técnica de no evitar la tormenta, no solicitar el llevar a cabo el aterrizaje por la otra pista del aeropuerto o alternar al
aeropuerto más próximo hasta que mejoren las condiciones meteorológicas.
4) La decisión de la tripulación técnica de penetrar la tormenta, habiendo sido ésta detectada en el radar meteorológico de la aeronave, con aproximadamente 190 millas de anticipación.
5) La pérdida de la visibilidad horizontal y vertical de la tripulación técnica al penetrar el núcleo de la tormenta (severa granizada). La severa
granizada ocasiona el opacamiento de las ventanas frontales de la aeronave y por consiguiente la pérdida total de la conciencia situacional."
