A crash involving a Canadair Challenger at Birmingham January 2002 that killed five was caused by the crew's failure to de-ice the wings before takeoff, accorinding to the report released by the AAIB. Possible impairment of crew performance by the combined effects of a non-prescription drug, jet-lag and fatigue were considered factors. (AAIB)
