Narrative:
PROBABLE CAUSE:AN INADEQUATE PERFORMED PREFLIGHT INSPECTION BY THE DUAL STUDENT AND THE INADEQUATE SUPERVISION OF THE DUAL STUDENT BY THE INSTRUCTOR PILOT. CONTAMINATED FUEL WAS A FACTOR IN THE ACCIDENT.
