The aircraft, a Cozy-Canard III, N718JT, crashed on take-off/departure approximately 1/4 mile from the airfield, Winslow-Lindbergh Regional Airport - KINW, Winslow, Arizona.
The aircraft was destroyed and the pilot received serious injuries. The passenger onboard was fatally injured.
Prior to takeoff, the pilot had mentioned a rough running engine.
