Written off 29 October 1994 when crashed during what was later presumed to be a CFIT (Controlled Flight Into Terrain) in the Waikukupa Valley, near Fox Glacier, Westland National Park, New Zealand. The helicopter collided with the side of the valley during a scenic flight, killing all seven aboard (pilot and six passengers). The TAIC investigation did not positively establish the cause but thought it was probable the aircraft inadvertently flew into cloud, causing spatial disorientation, and making pilot unaware of his proximity to the side of the valley.
Ex-VH-HRD; became ZK-HWV in November 1988. Registration ZK-HWV cancelled July 1995 as aircraft "destroyed".
