Rebuilt from OH-58A 72-21322. Written off (destroyed) 24 February 1991: CFIT (Controlled Flight Into Terrain) in dense fog, during combat operations as part of "Operation Desert Storm". Pilot - 2nd Lt Gary Stethens - injured, observer - 1st Lt. Michael Klee - injured.
