Descripción:
USA Jet Flight 199 departed Detroit-Willow Run Airport, MI (YIP) on July 5 for Hamilton (YHM), Canada to pick up cargo. Automotive parts were loaded on board and the flight continued to Shreveport (SHV) where it arrived at 23:19 CDT. After clearing customs it took off again at 23:48 CDT, bound for Saltillo (SLW), Mexico.
The DC-9 crashed in an industrial area 800 m north of the airport. The airplane broke up and burned.

Probable Cause:

CAUSA PROBABLE
La Dirección de Investigación de Accidentes e Incidentes de Aviación determina que las causas probables del accidente fueron la continuación de una aproxima~ión final inestable sin tener la pista a la vista y la consecuente pérdida de control - a baja altura- ante la inminencia del impacto.
FACTORES CONTRIBUYENTES
- Condiciones meteorológicas en el aeropuerto
- Aproximación no estabilizada
- Fatiga
- Falta de experiencia del copiloto
- No seguir procedimientos adecuados en la realización de las operaciones
- Falta de procedimientos operacionales
- No seguir procedimientos adecuados del controlador de torre Saltillo
- Falta de supervisión de la autoridad
- La coincidencia de factores que individualmente no representarían un sustancial aumento en el riesgo de las operaciones, pero que en este caso se sumaron, esto es; vuelo nocturno de madrugada, poca experiencia del copiloto, omisión de briefing por parte del capitán, un solo Jepessen para dos pilotos, DME #2 inoperativo, barras de comando capitán in operativas, arco DME, aproximación ILS/DME, omisión de reporte de bancos de niebla y finalmente saturación en las comunicaciones con el Centro de Control Monterrey.
