Narrative:The Jetstream 32 suffered a runway excursion during landing on runway 18 of Bahía Solano-José Celestino Mutis Airport, Colombia.
The aircraft sustained substantial damage to both propellers, engines and the rear of the fuselage, but the occupants were not hurt.
Probable Cause:
Probable cause(s):
Runway excursion as a result of a long landing, made after an unstable approach (low angle of approach and high speed).
Lack of situational awareness by the crew, by not recognising, or recognising and accepting, that they were undertaking an unstable approach to a limited aerodrome and not taking corrective action.
Poor decision making on the part of the crew in deciding to continue the approach and landing instead of going around when it was evident that they would make a long landing.
Contributing Factors:
Absence of a sterile cabin which reduced the situational awareness of the crew, during configuration and preparation of the aircraft for the final approach and landing.
Lack of supervision by the Air Operator, when scheduling the flight of the aircraft to a runway whose length was not sufficient for a safe operation, with the total weight it carried.
Poor conditions in the safety area of the Solano Bay airfield, which did not allow normal deceleration once the aircraft left the runway.

Aerocivil issued 6 Safety Recommendations
Issued: -- | To: SARPA | REC. 01-201841-1 |
Review the Company\'s Operations Manual and Standard Operating Procedures and include the concept and parameters of Stabilized Approach for each flight team and for each type of approach, also considering in particular, the parameters of Stabilized Approach in special airports, require their compliance and motivate the crews to apply the necessary corrective measures to get out of that condition, including the execution of overpass or missed approach. |
Issued: -- | To: SARPA | REC. 02-201841-1 |
Reinforce to the crews the knowledge and application of the Sterile Cabin concept, emphasizing the obligation to comply with this procedure to achieve a high situational awareness and avoid the omission of important information during approaches. |
Issued: -- | To: SARPA | REC. 03-201841-1 |
Review the Runway Analyses of the airfields where the Company operates, to determine the characteristics of each runway and establish the operating limitations (weight, weather conditions and others), for each type of aircraft. Prepare a risk analysis of aerodromes, applying the processes of the Operational Safety Management System. |
Issued: -- | To: Aerocivil | REC. 04-201841-1 |
Through the Secretariat of Operational Safety and Civil Aviation, manage before the Municipality of Bahía Solano, Operator of the José Celestino Mutis Aerodrome, the improvement of the operational conditions of the aerodrome, such as, among others: the frequent replacement of the windshields, the complete and correct demarcation of the runway, the cleaning of the runway surface, the adequacy of the safety zones and the prohibition of the transit of people and vehicles in the operational areas. |
Issued: -- | To: Aerocivil | REC. 05-201841-1 |
To arrange, through the Secretariat of Operational Systems, the installation of equipment, systems, instruments and other necessary elements so that it is possible to count on the information of winds, altimetric adjustment and temperatures, in the airport of the city of Bahía Solano. |
Issued: -- | To: Aerocivil | REC. 06-201841-1 |
Through the Secretariat of Operational Safety and Civil Aviation, to make this Investigation Report known to the Non Regular Aviation Operators, so that they implement the recommendations, as appropriate, and the Report is taken into account to improve the Operational Safety Management Systems. |
