Narrative:
A Piper PA-23-250 Aztec, N5675Y, hit a mountain at 4300 ft MSL, 22 miles north-west of Medford, Oregon.
The pilot was cleared for a straight-in ILS approach. He reported at the NDB, inbdound. However the NDB signal was out of tolerance.

Reportedly, the occupants died from exposure as the controllers were watching the Super Bowl on a TV and were unaware of the crash.
