According to the following extracts from the official NTSB report into the accident: "TAKE OFF MADE WITH ELEVATOR TRIM TAB POSITIONED TO THE NOSE HIGH POSITION; PILOT IN COMMAND - INADEQUATE PRE FLIGHT PREPARATION AND/OR PLANNING; FAILED TO OBTAIN/MAINTAIN FLYING SPEED"
