Narrative:
Written off (damaged beyond repair) May 31 1965 when crashed at McKenzie Bridge State Airport, three miles east of Mckenzie Bridge, Lane County, Oregon; aircraft flew into trees on final approach. Of the four persons on board (pilot and three passengers) the three passengers were uninjured, but the pilot sustained serious injuries.

According to the official NTSB report into the accident: "PILOT IN COMMAND - FAILED TO MAINTAIN DIRECTIONAL CONTROL; IMPROPER COMPENSATION FOR WIND CONDITIONS. DOWNWIND LANDING MADE BECAUSE OF AIRSTRIP ENVIRONMENT WHICH REQUIRED IT". (Because of the sloping runway at this airport, aircraft must approach and land to the East and take off to the West (Runway 06/24)).
