PROBABLE CAUSE:INADEQUATE PREFLIGHT PLANNING/PREPARATION, IN AS MUCH AS THE PERFORMANCE DATA WAS NOT USED BY THE PILOT IN COMMAND. FACTORS WERE THE HIGH DENSITY ALTITUDE, INADEQUATE AIRSPEED, AND PREMATURE LIFTOFF.
