PROBABLE CAUSE: THE PILOT'S DECISION TO LAND ON AN ICY RUNWAY AND THEN ATTEMPT TO TAKE OFF. CONTRIBUTING TO THE ACCIDENT WAS THE PILOT'S DECISION TO COMPLETELY RAISE THE FLAPS AFTER THE BALKED LANDING.
