Narrative:
Written off (destroyed) January 21, 1971 when crashed during the initial climb out phase from Lakefront Airport, 4 miles North East of New Orleans, Louisiana (at approximate co ordinates 30°02′33″N 090°01′42″W): aircraft computed to be 467 lbs over maximum gross take off weight. C of G found to be at extreme aft limits. Aircraft did a complete roll to the left before impacting into the ground. Both persons on board (pilot and one passenger) were killed. According to the following excerpt from the official NTSB report into the accident:

"PROBABLE CAUSE(S)
PILOT IN COMMAND - INADEQUATE PRE-FLIGHT PREPARATION AND/OR PLANNING
MISCELLANEOUS ACTS,CONDITIONS - IMPROPERLY LOADED AIRCRAFT-WEIGHT-AND/OR C. of G."
