Narrative:
Written off (destroyed) April 27, 1979 when crashed at Lorain County Airport, New Russia Township, 5 miles South West of Elyria, Ohio (at approximate co ordinates: 4120′39.4″N, 08210′39.5″W). Both crew - instructor and co pilot (under training) - were killed. According to the following excerpt from the official NTSB Report into the accident:

"PROBABLE CAUSE(S)
PILOT IN COMMAND - INADEQUATE SUPERVISION OF FLIGHT
CO PILOT - IMPROPER OPERATION OF FLIGHT CONTROLS - MISUSED OR FAILED TO USE FLAPS
FACTOR(S)
PILOT IN COMMAND - LACK OF FAMILIARITY WITH AIRCRAFT
MISCELLANEOUS ACTS,CONDITIONS - SIMULATED CONDITIONS"

Witnesses described seeing aircraft perform a go-around, with the flaps down, and with the engine 'windmilling'. Registration N82282 belatedly cancelled by the FAA on March 24, 2011 - 32 years after the accident
