Narrative:
F-4B Phantom Bu Aer No 148374 'AJ 105' of US Navy squadron VF-74 on the USS Forrestal (CVA-59). Written off on October 27 1962; crashed on take off from the USS Forrestall due to Attitude gyro malfunction, and flew into ocean. Pilot - not named - ejected safely, and was rescued by the "plane guard" helicopter. The RIO - Lt. W. C. Meintzer - also ejected, but was killed
