Narrative:
Issued to the Republic of Singapore Air force under FMS Program: Peace Carvin III. Delivered to the Singapore Air Force Training Unit at Luke AFB, Arizona, in February 1999. Despite the attachment to a USAF unit (425th FS, 56th OG), USAF markings (including LF tail code) and being based in the US, this F-16 was the property of the Singapore Air Force (leased from the US Government).

The crash occurred at about 21:36 hours PDT (Pacific Daylight Time), south of the Barry M. Goldwater Range near Sells, Arizona*, during night training flight. Cause of crash determined to be pilot error (either G-LOC or spatial disorientation). The F-16 is reported to have dived into the ground almost vertically from 14,000 feet, the pilot having failed to recover in time from "blacking out" due to G-LOC (G-induced Loss of Conciousness)

The pilot, Lt. Loo Kwang Han, did not eject, nor did he survive the crash.
