Narrative:
The Mil Mi-2 helicopter hit the water of Shosha River on final approach.

The most probable cause of the accident was the pilot-in-command’s (PIC) loss of altitude control during approach for landing at dusk over a calm water surface, which led to the helicopter colliding with the water while still under control (controlled flight into terrain).

A contributing factor to the accident was the PIC’s failure to switch on the radio altimeter before flight, in violation of the Mi-2 helicopter Flight Manual (AFM) requirements.
