Following a declared enroute diversion to the Kentucky Dam State Park Airport (M34), Gilbertsville, Kentucky, the aircraft impacted the wooded terrain about 10 miles short of the airport. The FAA said air traffic controllers lost contact with the Piper PA-34 after the pilot reported engine problems. Four of the five occupants onboard were fatally injured. One 7 year old occupant onboard the aircraft received unspecified injuries but walked to a nearby house to raise the alarm.

The National Transportation Safety Board determines the probable cause(s) of this accident as follows: The pilot's failure to properly set the left engine fuel selector before takeoff and to recognize the incorrect setting during the flight, which resulted in fuel starvation and a loss of engine power on both engines.
