A Bell 212 helicopter, transporting a worker suffering from heart attack, crashed shortly after its take off into the waters of the Persian Gulf. At least two occupants were killed, the patient and a physician assistant , Two are still missing. The pilot was saved.
Main cause: Human error due to lack of attention to attitude indicator/Spatial disorientation

According to several sources an Iranian helicopter has ditched into Persian Gulf when the helicopter was performing an offshore operation near Lavan Island.
We’re currently looking for more information.#Iran
The helicopter that involved the today’s accident is EP-HUA (no.1 in the picture), owned by Iranian Helicopter Services Company (ساخه) & operated on behalf of IOOC.
