Narrative:After landing at Jorge Enrique González Torres Airport (SJE/SKSJ), San Jose del Guaviare, the cargo Douglas DC-3D with only three crewmembers on board, veered off the runway, sustaining minimal damage.
After completing a non-scheduled cargo flight, the aircraft was returning Pacoa to San José del Guaviare without cargo and with three crew members on board. The aircraft followed a visual traffic pattern and approached as authorized by the Control Tower for runway 19.
Upon touchdown of the main landing gear wheels, the crew experienced a deviation from the centerline that they could not control due to the immediate loss of tire pressure on the left main landing gear wheel. The pilot attempted to control the aircraft; however, they lost control, and the aircraft veered off the runway to the left, into the safety area, where it finally came to a stop.
The aircraft came to rest at coordinates N02°34'48.49" W072°38'20.44", at an elevation of 187 meters, on the left side of the aerodrome safety area. The aircraft was located 620 meters from the threshold of runway 19 and 21 meters and 36 meters from the runway edge and centerline, respectively.
The crew confirmed the situation, shut down the engines, cut off electrical power, and began evacuation procedures. All three occupants exited the aircraft on their own, unharmed. No fuel leak occurred, and no fire ensued.
The accident occurred in visual weather conditions. During the low-speed runway excursion, the main landing gear fell into a ditch parallel to the runway, located 21 meters from the edge, where the propeller blades of both engines made contact with the ground.
The investigation determined that the accident was caused by the following probable cause:
- Loss of ground control due to damage to the left main landing gear tire caused by a screw on the runway, which punctured the tire, causing it to deflate and resulting in a loss of directional control and lateral runway excursion.
The contributing factors identified were:
- Absence of Standard Operating Procedures by the company to respond to various operational emergencies.
- Non-compliance with the published programs at Capitán Jorge Enrique González Torres Airport, serving the municipality of San José del Guaviare, for FOD control and wildlife management programs.
