A mass was being held at the official residence of the Governor of the Federal District. Before the event ended, a lady, the governor's sister, accompanied by a member of the Military Police of the Federal District, asked the pilot to take them to the governor's private residence to pick up the image of a saint, and then return. The flight would therefore consist of transporting these passengers from the official residence of the governor of the Federal District to his private residence. As requested, the aircraft took off from the ceremony site carrying the two passengers. The flight time was five minutes, and the landing went smoothly. Even though it was not approved, there was a windsock on the helipad, and the landing was made in such a way that it was not possible to keep the aircraft's exit door facing the entrance door of the residence. While still in the landing procedure, a man approached the aircraft from its right side, the same side where the pilot was at the controls of the helicopter, and attracted his attention. In the meantime, the landing was completed and the passenger disembarked through the left side door, heading quickly towards the rear of the aircraft. When trying to pass under the tail cone of the helicopter, she ran into the tail rotor blades that were in motion, and was fatally hit. The aircraft suffered minor damage.
