Narrative:
A Eurocopter EC 120B Colibri crashed while on final approach at Eliporto Cassina Rizzardi (Cassina Rizzardi Heliport) on the entrance ramp to the Autostrada dei Laghi (A9/E35) motorway near Fino Mornasco, Como, Lombardy.
The pilot sustained serious inhuries, while the passenger sustained minor injuries. The helicopter was damaged beyond repair.

The helicopter approached the heliport from the east. According to a witness, the helicopter was at an altitude of about 25 m (80 ft) when it started falling. The pilot reported he approached at an altitude of 20/30 m and started descend at an horizontal distance of 15/20 m from the helipad with a slight left yaw input to contrast the torque. He reported the pedal did not respond to the left yaw input and the helicopter started rotating clockwise. After 2 or 3 slow spins, the helicopter crashed on the entrance ramp. In the tests made after the crash, the flight controls function appeared normal. The pilot had a total of 750 flight hours on rotary wing, 150 of which on the EC120B.

The cause of the event was a loss of control of the helicopter in flight by the pilot, which occurred when the helicopter was at low altitude, in the vicinity of the target helipad, during pre-landing positioning. The safety investigation, however, was unable to determine with incontrovertible certainty which factor triggered the uncontrolled rotation of the helicopter, which then led to its crash. However, it cannot be ruled out that the pilot's limited experience on the type of helicopter in question, which had different characteristics to the one (NH500) on which he had gained most of his experience, may have contributed to the occurrence of the event.
