Narrative:LATAM Airlines flight LA4292, an Airbus A320-214, CC-BAS, suffered a nose gear tire failure during takeoff from Rionegro/Medellín-José María Córdova Airport (MDE) in Colombia.
The aircraft took off and the crew received some NLG failure indications. The crew decided then to perform a precautionary landing back at MDE. The aircraft made holdings to consume fuel and landed at about 20:20 UTC.
The nose gear was locked 90 degrees off-center, causing the tires to separate during the landing sequence. There were no injuries.
Probable causes
Execution of a takeoff maneuver with the nose gear wheels at 90º to the left of the aircraft longitudinal axis that caused damage to the nose gear system, and the execution of an emergency landing.
Discrepancy in voltage monitoring from the Brake and Steering Control System Unit (BSCU) to the Variable Rotation Differential Transformers (RVDT), due to a possible transient electrical discontinuity in the secondary voltages sent by the RVDT 3GC.
Contributing Factors
Disconnection by MEL of the NWS system due to previous malfunction of the Brake and Steering Control System Unit (BSCU).
Inability to monitor and verify in cockpit the permissible nose gear wheel angle limitations during control through asymmetric thrust and braking action during taxi.
