An Air Tractor AT-504 (HP-1955AT) and an Air Tractor AT-502 (HP-1966AT) were involved in a mid-air collision during aerial application activities at Finca 45 in Guabito, Changuinola district, and crashed.
Both pilots received serious injuries and later died of their injuries sustained in the accident.
Contributing factors
- Poor supervision by the operator on the application of the procedure for two or more aircraft that are spreading on the same plantation plot
- Lack of effectiveness of the Operational Safety Management System due to not feeling the obligation to implement the company's SMS, affected by lack of implementation and poor risk management and poor linkage between different processes.
