INADEQUATE PREFLIGHT PREPARATION AND/OR PLANNING, FUEL INJECTION SYSTEM, WATER IN FUEL, FAILED TO OBTAIN/MAINTAIN FLYING SPEED, IMPROPER EMERGENCY PROCEDURES.
