Narrative:
THE HELICOPTER CONTACTED THE PLATFORM WITH A LANDING SKID AND ROLLED OVER DURING AN ATTEMPTED TAKEOFF. AS THE HELICOPTERLIFTED OFF OF A TRUCK MOUNTED PLATFORM A SKID BECAME FOULED BY THE TRUCK AND A DYNAMIC ROLL-OVER OCCURRED.
