Written off (damaged beyond repair) when crashed at Hilversum Airfield, Hilversum, Gooi region, Province of North Holland 16.8.65; during a banner towing flight, aircraft made an emergency landing after a banner got stuck on the undercarriage while picking it up, and aircraft nosed over. According to a rough translation from Dutch into English of the accident report (see link #5 for the orignal Dutch text):

"16.08.1965
PH-NEB de Havilland DH82A Tiger Moth 85189
Hilversum
After picking up the advertising banner, it got tangled in the landing gear. The pilot immediately returned to land. The drag naturally snagged, and the drag was so strong that it did not break, so that the Tiger Moth was braked violently and immediately rolled over on touchdown. The damage was so severe that the aircraft was no longer repaired".

Registration PH-NEB cancelled 30.8.65 as "Withdrawn From Use"
