Narrative:On August 4, 2000, at 0920 universal coordinated time (UTC), a Bell 206B (registration ZS-HKP, Bell Helicopter serial no. 1426), that was privately owned and operated by the pilot, crashed on farmland near Burgersfort, South Africa. The daylight flight in visual meteorological conditions had departed another farm in the area, and was en route to a mine in Steelpoort, South Africa.
Initial reports are that after an engine difficulty, the pilot entered an autorotation, and the pilot and sole passenger on board were fatally injured in the impact and post-crash fire.
Probable Cause
The engine failed during flight and the pilot was unable to execute a successful autorotation. The cause of the engine failure could not be conclusively established.
