Narrative:Whilst filming a salt processing facility on the shores of the lake on behalf of the Great Salt Lake Minerals Co., the Helicopter flew into the surface of the water, cartwheeled and eventually came to rest in 4 feet of water some 20 feet from the shore line.
The helicopter had approached the location at 300 feet agl and had then gradually descended to about 50 feet agl over the water to begin the film run using a radar altimeter set at 50 feet for height guidance. However, during the run, the pilot apparently looked away for a moment and when he looked back, the helicopter had begun to lose height. The pilot attempted to arrest the descent but the helicopter hit the water before he could correct the situation.
At the time of the accident, conditions were calm, with no wind and the surface of the water was glassy, limiting the pilot's depth perception.
The NTSB determined the probably cause of the accident to be: The pilot's failure to maintain proper altitude. A factor to the accident was glassy water.
