F-16C 89-2059 of the 422nd Test Evaluation Squadron, 53rd Test Evaluation Group, USAF was written off on October 3rd 1991 when it crashed on the Nellis AFB Range, Clark County, Nevada. The pilot, Captain Percival Taylor "Teflon" Gates, was killed in crash.

89-2059 was delivered on October 31st 1990, so was just under one year old when it crashed.
