Narrative:
On 11/9/86, F-16A 80-0599 of the 16th TFTS, 57th FWW, piloted by Major Terry L. Slawinski was on a Fighter Weapons School upgrade sortie out of Nellis AFB, Nevada. Major Slawinski was the flight lead and instructor pilot. All aspects of the mission were normal, until departing the working area for recovery back at Nellis. At that point, the aircraft developed engine problems and caught fire. The pilot ejected successfully at 4,000 feet, and the aircraft was destroyed on impact.

Per eyewitness report from the pilot, Major Terry L. Slawinski:

"I remember the day well! It was a tough emergency to figure out. Glided, on fire, from 22,000 feet to about 4,000 feet, before I bailed out. Spent one hour on the ground before the chopper found me. I saw most of the big pieces in a hangar at Nellis, but I never saw the crash site"

The wreckage came down within a few yards of where F-86E 51-13044 had crashed on 8/1/1955
