Narrative:
Crashed just west of Mount Barrow during training mission. Pilot Capt Theodore S. Treadwell and Staff Sgt Roderick D. Nesmith killed. Incorrect installation of a component in the tail rotor, which caused a rivet to crack and resulted in the fin breaking off, leading to complete loss of control.
