Following a loss of engine power, the aircraft experienced an emergency autorotation and subsequent impact with heavily wooded mountainous terrain in Sacred Falls State Park on the island of Oahu. The helicopter came to rest partially inverted, sustaining substantial damage, and the four occupants onboard were not injured.

The Department of Land and Natural Resources said the helicopter was being contracted by the Division of Forestry and Wildlife for field work. The team onboard had just deployed seabird song meters in Kaluanui Natural Area Reserve as part of ongoing population studies when the chopper got into trouble.
