Narrative:On September 28, 1996, at 08:35 EDT (Eastern Daylight Time) a Mitsubishi MU-2B-30, N618BB, was substantially damaged during a precautionary landing at the Ross County Airport (RZT), Chillicothe, Ohio. The certificated airline transport pilot and three passengers were not injured. Instrument meteorological conditions prevailed for the passenger flight that originated at RZT, at 08:30 EDT. An instrument flight rules flight plan had been filed for the flight conducted under 14 CFR Part 135. According to a pilot statement, he departed runway 05, destined for Columbus, Ohio.
On take-off from Runway 05 at Chillicothe, as the aircraft climbed through about 300 feet agl. with the undercarriage retracted and flaps at 20 degrees, the left engine reportedly lost power. The pilot feathered the left propeller, applied full power on the right engine and then made a left turn to land back on Runway 23. The flaps initially remained at 20 degrees but were later extended to 40 degrees.
The pilot reported that, after the engine failure, he was unable to maintain altitude and had difficulty maintaining directional control, even with full right rudder. The aircraft subsequently touched down some two-thirds of the way along Runway 23 with its right main undercarriage off the side. The aircraft then veered further to the right, and its nose and right main undercarriage failed when it ran into a ditch.
As the aircraft came to rest a fire broke out on the right engine. The accident happened in daylight (08:35 EDT) and in VMC. Wind was 310 degrees/10 knots; temperature and dew point were 53 degrees F. The aircraft was operating an air taxi flight to South Bend.
An examination of the left engine revealed that the torque sensor housing had failed due to fatigue, resulting in a loss of drive to the fuel pump. On September 14, 1979 a Service Bulletin (SB) was issued for the replacement of the torque sensor housing with an improved housing. The manufacturer overhauled the engine on December 11, 1979 but the SB was not complied with. The SB indicated a history of resonant vibration causing cracks in the housing arm of original torque sensor and gear assemblies and that the housing should be replaced.
The investigation also found that the pilot did not comply with engine failure procedures and required air speeds and that the aircraft Flight Manual cautioned not to use 40 degrees of flaps during single engine landings. The NTSB determined that the probable cause of the accident was the failure of the pilot to follow the published emergency procedures after loss of power in the left engine.
Factors relating to the accident were: fatigue failure of the left torque sensor and gear assembly, which resulted
in the loss of engine power, failure of the manufacturer to comply with the respective service bulletin, and the pilot's improper use of the flaps and reverse (single-engine) thrust.
The fire continued to burn until CFR arrived approx 25 minutes later. The airframe was damaged beyond repair, and the registration N618BB was cancelled by the FAA on July 28, 1999 as "destroyed"
