On 10 January 2009 at 1030Z, the pilot departed from his farm (Kornkopie) to Lanseria Aerodrome on a Visual Flight Rules (VFR) flight. According to a family member that was with him on the farm prior to his departure, the weather conditions in the area of the farm were fine with some scattered clouds (3 to 4/8 {octas}) about. The aircraft was detected by Secondary
Surveillance Radar (SSR) abeam just south of Dwaalboom for a brief period. At the time, the aircraft was flying at a height of 6 500 feet above mean sea level (AMSL). Several minutes later it was again picked up by radar at a location 25 nm north-east of Pilanesburg Aerodrome (FAPN). It was noted that he was still on track towards Lanseria Aerodrome, but he had
descended to a height of 5 800 feet. At 1049Z, a short while later, the aircraft disappeared from radar at a distance of 20.6 nm on a heading of 042°M (magnetic) from FAPN. It was noted that the aircraft had impacted with level (bush type) terrain in a left-wing low attitude at an angle of approximately 90°. (The impact angle should be regard as an estimate, as all
ground impact markings were disturbed (washed away) by recent rain that had fallen in the area). The pilot was fatally injured in the accident. The aircraft was located after four days, due to poor visibility and rain in the area.
Probable Cause
The pilot continued the flight into adverse weather conditions and became disorientated with the result that the aircraft entered a graveyard spiral and impacted terrain.
Contributing factors:
The pilot was not rated for flight into IMC conditions.
